We May Use and Disclose Your Health Information in the Following Ways:

We also may use and disclose your identifiable financial and health information to obtain payment from third parties who may be responsible for payment for our services and related costs (such as family members). Any authorization you provide regarding the use and disclosure of your identifiable health information may be revoked on a prospective basis at any time in writing. We will retain your health information for the period mandated by law from time to time and in the absence of a period mandated by law for certain information, we have the right, but not the obligation, to dispose of such information any time following the period of 12 months from the date of the later of (i) the date you last received services from us and (ii) the date of termination of any Service agreement you entered into with us.
