What information do we collect ?
This site recognizes and collects, when possible, the domain name of a visitor’s server. We do not automatically collect the full e-mail address of visitors to our website. The only way we obtain your name or e-mail address is when you choose to provide that information to us.
How do we use the information we collect ?
MSG does not sell, trade or rent personal information about its website visitors.
The information we collect about the domain name of the server from which you are visiting is aggregated and used solely to determine the number of visitors to our site each day, and sources of traffic.

We May Use and Disclose Your Health Information in the Following Ways:
Provision of Services. We will use your identifiable health information in order to provide the services you have purchased from us. The MSG staff may use or disclose your identifiable health information in order to provide the services to you, including for example disclosing information to your medical coverage provider in order to arrange for you to receive certain medical appointments or services. We may use and disclose your identifiable health information to contact you and remind you of visits/appointments/need for medication.
Release of Information to Third Parties. Additionally, we may disclose your identifiable health information to others who may assist in your care (such as your physician, therapists) and we may release your identifiable health information to individuals that you designate in the PRIVACY WAIVER AUTHORIZING DISCLOSURE OF MEDICA RECORDS TO THIRD PARTIES for that you provide to us.
Payment. We may use and disclose your identifiable financial and health information in order to bill and collect payment for the services you may receive from us. We also may use and disclose your identifiable financial and health information to obtain payment from third parties who may be responsible for payment for our services and related costs (such as family members). We may use and disclose your identifiable health information to inform you of health-related benefits and services that may be of interest to you.
Business Operations. We may use and disclose your identifiable health information to operate our business such as to evaluate the services we provide, improvement of services, expansion of services offered and to conduct cost-management and business planning activities.

We will obtain your prior written authorization for any use or disclosure that is not identified by this policy or permitted by applicable law. Any authorization you provide regarding the use and disclosure of your identifiable health information may be revoked on a prospective basis at any time in writing. After you revoke your authorization, we will no longer use or disclose your identifiable health information for the reasons described in the authorization.
Record Retention
We will retain your health information for the period mandated by law from time to time and in the absence of a period mandated by law for certain information, we have the right, but not the obligation, to dispose of such information any time following the period of 12 months from the date of the later of (i) the date you last received services from us and (ii) the date of termination of any Service agreement you entered into with us.
